Preparing for Your First Appointment

What to Bring:

Preparing for your first appointment at UC Davis Comprehensive Cancer Center? Here's what you need to know to make the most of your visit:

First appointment
  • Notes detailing your personal health history, including dates for:
    • Past and present health problems
    • Surgeries or procedures
    • Test results (if available)
    • Any known allergies
    • Vaccination records
    • Supplements, vitamins, and over-the-counter medications you're taking
  • Health information about your immediate family (parents, grandparents, siblings)
  • A list of questions for your healthcare team, with the most crucial ones highlighted
  • Any new symptoms you've noticed

Additional Tips:

  • Consider bringing a family member or friend to help take notes during your appointment
  • Don't forget your insurance card and identification
  • If applicable, bring a copy of your advance directive or durable power of attorney for health decisions
